Area Based Scheme questionnaire prize draw

Terms and Conditions

1) This draw is open to those who are signed-up to an Area Based Scheme (ABS) project and who have completed an ABS pre-install, post-install or follow-up questionnaire.

2) Entrants must be aged 16 or over and cannot be an employee of CHANGEWORKS Resources for Life Ltd. and its subsidiary companies, their families or any other organisation directly connected with the administration of the draw.

3) Entry into the competition is made by completing an ABS questionnaire.

4) Entry is optional – questionnaire respondents opt in by completing the prize draw question and submitting their phone number and/or email address.

5) Only one entry per household per questionnaire.

6) The ABS questionnaires are drawn quarterly, and include all Pre, Post-install and follow up questionnaires that are completed in that period. Each quarter covers:

• Q1 April, May, June
• Q2 July, August, September
• Q3 October November, December
• Q4 January, February, March

7) The prize draws will be made on no later than 1 month following final submission deadline.

8) Three winners per quarter will be selected at random and contacted by phone or email, with vouchers emailed to the winners.

9) The prize consists of one of three £100 Love to Shop vouchers. No alternative prize or cash equivalent is available.

10) Changeworks’ decision is final.
